Can I schedule my appointment to take place over tele-health?
Sometimes, we are able to schedule appointments to take place over tele-health. This is dependent upon the type of appointment you would like to schedule.

Which health insurance plans does the office accept?
Our office is in network with most plans from the following health insurance companies:
- Aetna
- Anthem BCBS
- Cigna / Evernorth
- Emblem Health GHI (Mental health benefits managed by Carelon)
- Fidelis Care
- Healthfirst
- Magnacare
- Medicare (We accept any secondary insurance with Medicare)
- Medicaid (Both private managed Medicaid plans and NY State Medicaid plans)
- NYSHIP (Mental health benefits managed by Carelon)
- United Healthcare (Commercial plans, Community plan, and Oxford plans)
- 1199 SEIU

Can I schedule a psychotherapy appointment with Dr. Weber, specifically?
At this time, Dr. Weber herself is unable to accept new patients for psychotherapy. There currently is not a wait list available to see Dr. Weber for psychotherapy appointments.
Why do I need a referral for my appointment?
Our office's policy is that a referral order from another healthcare provider is required in order to schedule either a neuropsychological evaluation or a psychological evaluation for bariatric surgery. This is regardless of whether or not your health insurance plan typically requires referrals for services.
FOR NEUROPSYCHOLOGICAL EVALUATIONS: All health insurance companies within the United States follow guidelines established by the Center for Medicare & Medicaid Services (CMS).
In order to establish medical necessity for neuropsychological evaluations, our office must be able to prove that this evaluation is required to create a new healthcare plan or adjust an existing healthcare plan. The CMS states in article A57481 that "Diagnostic procedures that have no impact on a patient’s plan of care or have no effect on treatment are not medically necessary," therefore another healthcare provider outside of our practice must order this service to be provided to you in order to ensure medical necessity.

Who can I get a referral order for a neuropsychological evaluation from?
We can accept referral orders from the following types of providers:
We cannot accept referral orders from:
- Neurologist
- Psychiatrist
- Primary Care Provider
- Nurse Practitioner (NP / PMHNP / FNP-BC)
- Physician's Assistant (PA)
We cannot accept referral orders from:
- Psychologists (PhD, PsyD)
- Social Workers (LCSW, LMSW)
- Psychotherapists (LMHC, LMFT, LCAT, LP)
How much will my appointment cost if I pay out of pocket?
Our office's out of pocket rates for different appointment types are as follows:
- Neuropsychological Evaluation: $3,500 total for consultation, testing, and follow up appointment
- Psychological Evaluation for Bariatric Surgery: $350
- Psychotherapy with a Limited Permit Therapist: $65 per session
Why does the office need a payment card on file from me?
Our office has established a new policy for appointments that are missed without any notice or cancelled less than 24 hours in advance. Our office typically books appointments approximately 1-2 months in advance, and appointments being cancelled with little to no notice in advance create difficulty for our office to be able to fill the appointment spots. Therefore, we are now charging fees for late cancellations and missed appointments.
We kindly ask that payment card information be provided to our office in order to book an appointment with us. All payment card information will be stored in our payment software, Square, and we can ensure that only our office staff will have access to payment card information. Upon completion of your final appointment with us, the office will immediately delete your payment card information from Square.

- Missed Psychotherapy: Equivalent of one co-payment, $25 if no copay
