**Please note our new office policy regarding late cancellations/no-shows.**

We now require a payment card to be kept on file in order to schedule an appointment. All patients will be charged a fee for appointments that are either missed or cancelled less than 24 hours in advance. 

Missed Neuropsychological Evaluation: $100.00
Missed Neuropsychological Evaluation Follow-Up: Equivalent of one co-payment, $25.00 if no copay
Missed Bariatric Evaluation: $50.00
Missed Psychotherapy: Equivalent of one co-payment, $25.00 if no copay
